Answer To Question 1: The Trust does use exception reporting which is electronically submitted, reviewed and outcomed via Allocate software. | |
Question Number 2: If so, how many exception reports were logged as raising an immediate safety concern in the a) 2022/23 and b) 2023/24 financial years? | |
